What do I need to do before my first appointment as a new patient?

As a new patient, if you've already made an appointment at our office please ensure that you've filled out your new patient paperwork prior to your first appointment. Please also print and sign both the appointment reminder and record release forms (located on the "What to Expect" page) and bring along your ID, insurance card, and a valid payment method.
